Submitting a Public Complaint Form

1. To fill out the form, press TAB on your keyboard to automatically be taken to each section that need to be completed by you.
2. Type in all the requested information in all fields.
3. On the Adobe Acrobat toolbar, select SAVE A COPY.
4. Save the Public Complaint Form to your computer.
5. Submit the Public Complaint Form in one of the following ways:
   1. Send an email to Code Enforcement and attach the Public Complaint Form to your email (email: sandrews@cityofcalimesa.net ).
   2. Print the Public Complaint Form and fax it to (909) 795-6187.
   3. Print the Public Complaint Form and either mail or personally deliver it to:
